Hillsborough Middle School Fall Sports Physical / Health Forms

6th Grade Parents/Guardians,

If your child plans to participate in a fall sport at HMS, a new physical is due prior to August 21, 2020.  Please check the health office website for upcoming information on how to submit your physical or health update form.  Please do not mail any sports paperwork to the health office.  More information to come soon due to current circumstances. 

In a proactive effort to reduce the stress of due dates, tryouts, and the difficulty of obtaining a doctor's appointment, we are sending this notice to parents to give ample time for completion of any required forms.  Any newly submitted physical must be less than 365 days old (as of the start date of the sport) or a new physical must be completed by a physician.  If the physical is more than 90 days old, a completed health update form is also required from a parent/guardian. Please check with your child’s physician for questions related to when their last physical was completed.
